Welcome to The Swallow Assessment Clinic. By accessing and using our services, you agree to the following Terms and Conditions. Please read them carefully before making any appointments or engaging in services at our clinic.

1. Appointment Booking
Appointments can be booked via phone or email. A confirmed appointment is considered an agreement to these Terms and Conditions.

2. Cancellation and No-Show Policy
We understand that life can be unpredictable. If you need to cancel or reschedule an appointment, please notify us at least 48 hours in advance to avoid a cancellation fee. To make a cancellation please call or email us. Please include the client’s name, the date and time of the session that you are cancelling. The time of your correspondence will be noted to determine if the late cancellation policy will be applied.

Cancellation Notice: A minimum of 48 hours' notice is required for cancellations or rescheduling.

Late Cancellations and No-Shows: A fee will be charged for any cancellations or no-shows made less than 48 hours before the appointment. This fee helps us manage our schedule and offer time to other clients.

Cancellation within 48hrs of the appointment will incur a fee of 50% of the appointment cost.

Cancellation within 24hrs of the appointment will incur a fee of 100% of the appointment cost.

Cancellation fees need to be paid prior to the next scheduled appointment.

Emergency Situations: We recognise that emergencies happen. If you have an emergency or are unable to attend due to unforeseen circumstances, please contact us as soon as possible, and we will review the situation on a case-by-case basis.

3. Payment Policy

Payment Methods: We accept payment via credit card, debit card, bank transfer and cash. Payment is required at the time of the appointment unless prior arrangements have been made.

Insurance Claims: We do not directly bill insurance companies. However, if applicable, we will provide you with a receipt that you can submit to your insurance provider for reimbursement. It is your responsibility to check with your insurance company regarding coverage for speech pathology services.

Outstanding Payments: Any outstanding payments must be settled before further appointments can be booked. A reminder will be sent if payment is not received within 5 days of the appointment.

4. Fees and Charges

Standard Fees: Our fees may vary depending on the type of service provided (e.g., initial assessment, therapy sessions, etc.).

Additional Charges: Any additional services such as report writing, travel, or extended session time may incur additional charges. These will be discussed with you in advance.

5. Privacy and Confidentiality
We are committed to maintaining the confidentiality of your personal and health information. All information shared during your sessions is protected in accordance with the Privacy Act 1988. 6. Client Responsibilities

Accurate Information: You are responsible for providing accurate information regarding your medical history, current health condition, and any relevant treatment information.

Active Participation: We encourage active participation in the therapy process, including following recommended therapy practices at home.

Respectful Conduct: We expect all clients to engage with our staff in a professional and respectful manner. Abusive or inappropriate behaviour will not be tolerated and may result in the termination of services.

7. Liability and Indemnity
While we strive to provide the highest standard of care, all treatments and services provided by the clinic are based on the information available at the time and the professional judgment of our clinicians. We do not accept liability for outcomes beyond our control, including any deterioration in health or well-being resulting from non-compliance with treatment recommendations.

8. Limitation of Services
Our clinic provides speech pathology services for adults (16+ years) with swallowing disorders +/- a concurrent communication disorder. We do not provide emergency medical services, and our staff are not qualified to manage acute medical conditions. If you require urgent medical attention, please contact a healthcare provider or emergency services immediately.

9. Clinic Hours and Availability
Our clinic operates on a part-time basis, with limited availability due to other professional commitments. Our hours and days of operation are subject to change. Please refer to our website or contact us directly to confirm current availability. We may occasionally close for public holidays or unforeseen circumstances, and we will notify you in advance if your appointment is affected.

10. Changes to Terms and Conditions
The Swallow Assessment Clinic reserves the right to update or modify these Terms and Conditions at any time. Any changes will be communicated through our website or via email. By continuing to access our services after such changes, you agree to the updated Terms and Conditions.
